Short DescriptionNurse Navigator I - Adult HealthThe Ambulatory
Adult Health Nurse Navigator is a professional care-giver who
partners with a multidisciplinary health care team consisting of
Digestive Health, Pulmonary Medicine, Nephrology and Endocrinology
specialists. The RN in this position is responsible for non-patient
facing nursing care that includes telephone triage, care
coordination of complex conditions and partnering with provider
colleagues in the provision of care excellence. Additionally, this
RN serves as resource to LPNs in their provision of non-patient
facing care.Excellent customer service, interpersonal and clinical
skills are required.Experience in ambulatory specialty care
preferred
