Description:Salary - $48,000 to $50,000Full COVID-19 vaccination
requiredThe Parents as Teachers Parent Educator (PAT PE) is
responsible for initiating and maintaining regular, long-term
contact with families through home visits. During these visits PAT
PEs concentrate on family-centered, strength-based, goal-oriented
services to assist families with issues involving the parent-child
relationship, improving the family support system, family problem
solving, and advocating for the client. Referral and monitoring of
health care usage and child development are also stressed.Essential
Functions:Travels door-to-door or site-to-site to locate and
communicate with pregnant women, directs them to needed services,
and encourages them to obtain prenatal careContacts women by
telephone, mail or home visits to assure proper follow through in
making prenatal visits and in obtaining other servicesSupports
clients to plan for their own prenatal services, social services,
housing needs, and job opportunitiesEducates clients about
parenting and prenatal care related topicsSchedules prenatal,
postpartum and well community resources needed by clientsMaintains
relationship with identified clients and provides emotional and/or
maternal support as neededRefers families needing more intensive
follow-up to appropriate agenciesKeeps careful records of all
contact made and follow-up as necessary. Keep accurate records of
services received by clientsEnsures confidentiality of client
records, sharing pertinent information only with the client's
approvalMeets with Supervisor regularly to report and discuss
workAttends initial and ongoing training sessions on maternal and
child health, welfare, housing nutrition, substance abuse,
counseling, parenting, infant and family support issuesAttends
community meetings, health fairs and represents SNJPC as requested
by SupervisorPerforms other duties as
assignedRequirements:Associate Degree or participation in courses
towards an Associate Degree or completion of a Community Healthcare
Worker Certification courseKnowledgeable about community
resourcesAbility to work well with a variety of people and develop
rapport with clients from varied cultural/ethnic backgroundsDesire
to expand knowledge baseCommitted to better health care in
communityEffective oral communication skills; Bilingual strongly
preferred; ability to keep written recordsValid driver's license,
car and current insurance policyPhysical Requirements:Must be able
to see to read documents and hear well enough to communicate with
othersMust be able to write, type on a computer, and use a
telephoneMust be able to operate a motor vehicleMust be able to
lift up to 10 poundsAdditional requirements to be determined with

traffic, weather, and other environmental conditions may be
