IIPosition Summary: The Case Managerpossesses the knowledge, skills
and experience necessary to perform case management activities
competently.The Case Manager completes a comprehensive assessment
and develops a Family Case Plan for each client. This position
works with clients to link them to appropriate care and resources
in the community in order to meet identified needs. The Case
Manager will possess a thorough knowledge of community services and
resources in order to facilitate referrals. This position will also
possess the skills, knowledge and experience needed to assist and
engage clients when entering care and when transitioning between
levels of care. The Case Manager will act as an advocate for their
clients.Duties and Responsibilities:Assists in the completion of a
comprehensive case management assessment for each client.Develops
an integrated Family Case Plan for each client.Links clients to
appropriate care and resources in the community, including
resources that address specialized needs such as agencies offering
services related to HIV/AIDS; mental health disorders; chronic and
acute health problems; and problems related to criminal justice
issues.Promotes the client's recovery to their fullest potential by
facilitating the assessment of skills, including literacy,
educational opportunities, job readiness skills, vocational
training, employment placement, and job retention and career
development.Offers each assigned client at least one (1) hour per
week of face-to-face time.Refers clients and assists them with
accessing substance use disorder treatment as needed.Assists and
engages the client with transitioning from one level of care to
another to ensure continuity of care.Coordinates interventions and
services provided by multiple agencies.Informs clients about needed
services, such as medical, legal, educational, vocational, and
employment services; and assists the client in accessing needed
services.Uses engagement skills to motivate and encourage client to
voluntarily access services that would assist them in maintaining a
healthy life-style.Advocates on behalf of the client.Provides
education related to HIV and hepatitis.Provides training on
recognition of relapse triggers and strategies to deal with
avoiding relapse.Participates with team members in the updating of
the Family Case Plan within established time frames or as indicated
by changes in client needs.Demonstrates knowledge of crisis
intervention and behavior management of aggressive
behavior.Involves clients and families in the service planning
process.Assists in the development and implementation of
educational programs.Demonstrates knowledge of community
resources.Accurately documents all client interaction within
established time frames. Documentation includes noting that each
assigned client was offered at least one (1) hour/week of
face-to-face time and whether that visit was accepted.
Documentation also includes the clients participation in any
contact and any services provided. .Writes in a professional and
competent manner.Maintains a valid NJ driver's license.Practices
safe and courteous driving habits.Provides services to
clients/families in their native language or is able to procure the
services of a translator.Works well with other staff and
supervisors.Demonstrates a willingness to serve on professional
committees, attend staff meetings, attend in-services, etc.Seeks
supervision in a professional manner and documents
supervision.Participates with team members and clients/families in
discharge planning.Provides services within state mandated scope of
services.Provides competent psychosocial education to
clients.Develops an adequate professional relationship with all
referral sources and relevant community agencies.Adequately
responds to the needs of clients, families and referral sources.As
appropriate to scope of practice, assists in the data collection
for each assigned client, including the following:The relationship
of each clients physical status to dependence.The nature of the
comparison to use alcohol and/or other drugs.The intensity of the
client's mental preoccupation with using alcohol and/or drugs.As
appropriate to scope of practice, completes urine tests according
to established procedures.As appropriate to scope of practice,
demonstrates a knowledge of the natural history of dependence.As
appropriate to scope of practice, demonstrates an understanding of
the bio psychosocial influences and effects of
dependencies.Documentation must be completed within one (1)
calendar day of service delivery.Provides on=call coverage.Minimal
Qualifications:Education: Bachelor's Degree in behavioral health
fieldLicensure: Valid Driver's License required. Tracking or in
active pursuit of CADC, LCADC certification at time of hire or
transfer..Experience: Three (3) years' experience working with
clients diagnosed with substance use disorderPreferred
Qualifications: Education:Master's degree in health, psychology,
social work education, or other behavioral health field.Equipment
Use: Computer, copier, facsimile, motor vehicleThe above statement
