Cooper University Health Care's Post-Acute Care Program is
responsible for managing the daily operations of the Population
Health Post-Acute Care Program including clinical quality, quality
improvement, and administrative responsibilities. - - - The manager
engages High performing post-acute partners, providers, leaders,
and clinical team members to establish and maintain relationships.
- They facilitate quality patient care, care transitions, and
engagement of patients/caregiver during the patient's admission to
Post-Acute High Performing Partner facilities. - - They also
partner with Cooper University Health Care team to identify areas
of growth and opportunity and facilitate program expansion and
optimization. - - -Engages High performing post-acute partners,
providers, leaders, and clinical team members to establish and
maintain relationships and communication through direct engagement
and on-site visitsFacilitate quality patient care, care
transitions, and engagement of patient/caregiver during patients'
admission to Post-Acute High Performing Partner facilities through
care coordination and collaborationPartner with CUHC team members
to collect, analyze, and report out program data/trends and make
recommendations for process improvement based on resultsResource to
patients, care givers, clinical and administrative teams regarding
CUHC Post-Acute ProgramPerforms other responsibilities as
