Job Description:
Our client, a reputable Financial Management Service provider,
operating since the - late 90s, is committed to the well-being of
their medical participants. Through a - self-direction support
program and service models, they strive to provide comprehensive
care within the comfort of individuals' homes. They are actively
looking to bring on Care Managers who are passionate about making a
difference in people's lives. Salary/Hourly Rate: $22.50/hr
Position Overview: The Care Manager will offer educational
resources to support and educate - participants eligible for
Medicaid or individuals with disabilities. This role is -
responsible for providing all essential resources to participants
to ensure they are - secure in remaining in their community and
home. It's important to note that this is a temporary-to-permanent
position. During the temporary phase, the Care Manager will work 40
hours per week and is not eligible for overtime. This role is 70%
driving to participants homes and 30% work from home.
Responsibilities of the Care Manager:
- Manage up to 145 caseloads, conducting 45-55 in-home visits
monthly
- Provide overall participant support and aiding in community
resource location
- Manage a high caseload, prioritizing exceptional client
service
- Maintain daily communication via phone and email
- Assist with monthly budgeting for program services and support
direct care - workers with paperwork
- Track participants spending plans, acting as a household
employer for direct care workers
- Enroll participants and complete necessary paperwork
- Analyze daily participant health, adhering to NASW
documentation standards
- Take immediate action in reporting complaints, abuse, neglect,
and - exploitation Qualifications for the Care Manager:
- Requires flexibility for frequent travel within a 60-mile
radius (70% of the - time)
- 2+ years of relevant experience, including knowledge of
Medicaid services and working with individuals with disabilities or
the elderly
- Strong technical skills; proficient in Microsoft Office,
especially Excel
- High-speed internet and dedicated home workspace required
- Valid driver's license, access to a vehicle, and active vehicle
registration/insurance necessary
- Must provide own transportation; travel is a requirement
- Excellent communication and problem-solving skills
- Highly empathetic with a confidential nature
- Independent worker with strong initiative and schedule
management
- Bilingual candidates preferred
- Strong customer service and people skills essential
- Case management experience is a plus
- Familiarity with the Personal Preference Program, Medicaid,
and/or Medicare regulations is beneficial Education Requirements:
- Bachelor's degree is strongly preferred
- Education can be substituted for 4+ years of related experience
