We are searching for Nurse Practitioners who will provide on-going medical management for patients in the post-acute care, rehabilitation setting. The Nurse Practitioner will be a member of the medical and interdisciplinary team to effectively designate processes for meeting the patient's medical, psychosocial, and spiritual needs.
