Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.PurposeJob DescriptionUnder the general supervision of the Chief Quality Officers' Office and with considerable latitude for the exercise of independent initiative and judgment, the Patient Safety Officer aligns with our High Reliability Organizational standards.Specific Duties And ResponsibilitiesThe Patient Safety Officer is charged with and responsible for the following:

  • Serve as a subject matter expert at South Brooklyn Health's Patient Safety Programs.
  • Collaborate with quality to achieve top performance metrics as measured by HEDIS, CMS, HRSA, and other regulatory agencies.
  • Oversee the culture of Patient Safety, compliance, and improvement initiatives.
  • Drives the strategic direction and oversight of the Internal Process Compliance function.
  • Safety Event Reporting: Track, trend and manage event reporting for "near misses" or "good catches"; implement and maintain a "good catch" celebration program.
  • Root Cause Analysis: Participate in or lead root cause analysis discussions; participate in performance improvement for corrective action plans
  • Leapfrog: Coordinate data collection and entry for Leap Frog twice a year along with Quality department; identify and participate in performance improvement projects related to low scoring areas of Leapfrog
  • Leads the Culture of Safety Survey: Organize distribution of the survey every two years; categorize the results by department and discipline to share with executive leadership, department leadership and front-line staff; track action plans
  • Ensures the alignment of National Patient Safety Goals: Track and trend data/improvements for National Patient Safety goals on quarterly basis at a minimum; regularly share updates in trends to executive leadership; utilize recognized/standardized tools for analysis such as run charts etc. and approved methods of performance improvement i.e. PDSA.
  • Leads Patient Safety Meetings: Attend monthly System-Wide Patient Safety Council meetings; Leads in house regular (monthly/quarterly) PS meetings at this facility; review reported incidents, trends of national patient safety goals, good catches, and any other pertinent projects
  • Leads Patient Safety Rounding: Participate in and coordinate Executive Leadership Rounds on a regular basis (weekly-monthly); collect, track, and coordinate action plans for shared patient safety concerns; share PS improvements regularly with front-line staff
  • Leads Patient Safety Education: Complete TEAMSTEPPS training for new employee orientation or for requesting units; regularly educate staff on National Patient Safety Goals, how to report safety events and the importance of reporting
  • Leads Patient Safety Projects: Help mentor and sponsor patient safety projects and encourage submission to conferences such as IHI or for awards such as HANYS Pinnacle AwardMinimum Qualifications
    • Graduation from an approved School of Medicine and completion of an approved residency and Board eligible; and,
    • N.Y. State license; and,
    • Five years of full-time paid experience in a responsible medical and clinical administrative and/or managerial capacity; or,
    • A Master's Degree from an accredited college or university in Hospital Administration, Health Care Planning, Public Health, Administrative Medicine, Business Administration, Public Administration or approved related program; and,
    • Eight years of progressively responsible experience in health and medical services administration, of which at least four years must have been in a managerial or administrative capacity.Department Preferences

      • Ability to assess all work and prioritize as necessary to meet reporting timeframes and deadline
      • Knowledge of and ability to perform Statistical review and analysis for quality improvement
      • Knowledge of and ability to perform Record Review
