Gallagher MAJOR FUNCTIONS: The Case Manager is responsible for
providing counseling and case management services related to
substance abuse, legal, mental health, medical and
vocational/educational issues. They work directly with the clients
in individual and group counseling sessions on issues surrounding
chemical abuse/dependency and advocates a drug-free lifestyle. The
Case Manager works closely with the rest of the staff as an
integral part of the clinical team. SPECIFIC DUTIES &
RESPONSIBILITIES: Provide individual counseling and case management
services addressing all medical, psychiatric, housing and
vocational concerns. In collaboration with each client, formulate
treatment plan, monitor achievement of objectives, review and
update treatment plan in accordance with regulatory requirements
and as clinically indicated. Maintain up-to-date client clinical
records including, at minimum, treatment plans, group counseling
notes and progress notes in accordance with regulatory guidelines
and program standards. Conduct education/prevention groups to
address topics which address chemical dependency and related
issues. Provide crisis intervention services. Responsible for
ongoing client discharge planning. Perform room inspections and
addresses client’s needs in relation to successful completion of
Activities of Daily Living (ADL), including money management,
medication management, and personal hygiene and other life skills
which support recovery. Provide other therapeutic services through
active participation, observation and evaluation of each assigned
clients engagement in the resident hierarchy, job functions, and
social and recreational activities. Facilitate house and community
meetings including morning & evening meeting, etc. Operate as a
liaison with all referral agencies providing regular verbal and
written progress reports. Establish community and agency
affiliations which address client needs and strengthen the delivery
of services. Provide education and direct services to family
members, as appropriate, in regards to resident’s substance abuse
and related behavioral issues. When necessary, conduct client
screening for contraband and collection of samples for urine
toxicology testing. Attend regularly scheduled staff meetings and
case conferences. Attend all required in-service training seminars.
Participate in quality improvement activities. Other relevant
duties as required. REQUIREMENTS: EDUCATION/TRAINING AND EXPERIENCE
KNOWLEDGE, SKILLS AND ABILITIES High School Diploma/GED required.
Familiarity with computer operation (Microsoft Word, AWARDS,
Outlook programs) preferred. Two (2) year’s experience working as a
case manager in a residential setting preferred. CASAC-T preferred.
Must be able to work a flexible schedule. Odyssey House is an equal
