Registered Nurse (RN) | Home Health Location: New York City, NY Agency: Cynet Health Pay: $2,352 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 260 Weeks Start Date: 4/2/2026 About the Position Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 days Shift: Day Hours per Shift: 8 Experience: Minimum 2 years in a home setting License: Current RN Licensure in New York State Certifications: BLS required before the start date Must-Have: - Two years of experience in a home setting within the last ten years. - Recent experience conducting comprehensive assessments of patients. Description: Required Licensure: Current RN Licensure in New York State. Required Experience: Two years of experience in a home setting within the last ten years. This experience may include working in the field for a certified home health agency, licensed home care service agency, or managed long-term care. Nurses must conduct assessments similar to UAS, such as OASIS. Providing primary care to homebound persons is essential. Experience in care management for persons receiving home and community-based services is required. Home care experience as a paraprofessional or LPN caring for a minimum of two patients per week is acceptable. Preferred qualifications include experience conducting assessments in UAS, especially within the last 365 days. Experience caring for a geriatric population and managing chronic diseases in a home setting is preferred. Candidates must indicate the class start when submitting their resume. All candidates should be prepared to conduct hybrid telehealth and in-field assessments. Candidates cannot undertake UAS assessment assignments elsewhere. Job responsibilities include: - Performing initial in-home pre-evaluations of patients, including evaluation of health status, strengths, care needs, and preferences. - Reviewing medical documentation and health referral forms relevant to cases. - Entering evaluation data into electronic forms and transmitting as required. - Documenting concerns or other issues that surface during assessments. - Emphasizing continuity of care to reduce fragmentation, duplication, and gaps in treatment plans. - Discussing health care options and available supports with consumers. - Facilitating communication between the call center and eligible consumers for enrollment into managed care. - Enhancing communication and collaborative relationships with interdisciplinary care team members to improve care coordination. - Collecting quality review data and documenting case notes. - Identifying opportunities for health promotion and illness prevention. - Maintaining knowledge of community resources and payer requirements. - Meeting all performance standards established for the position. - Performing other duties as assigned by the Regional Nurse Manager or project management.
