advocacy. Role Overview The Per Diem Physician is a key member of
an integrated health care team and instrumental to its success.
This position provides primary medical care services and health
education/screening activities to meet the healthcare needs of the
Project HOME community, Project HOME residents, neighbors in the
community, homeless persons & staff. Per Diem Physicians can work
anywhere between 4-30 hours, based on availability, during the
various sessions Monday-Friday. Sessions are 8:30am-12:30pm and
12:30pm-4:30pm. Key Job Responsibilities Provides direct patient
care at Project HOME, which include: Primary care medical services
for children and adults, and pregnant women. Competency in basic
office procedures and LARC placement. Teach medical students and
precept residents as required. Collaboration with Project HOME
staff and behavioral health staff on the healthcare needs of
Project HOME residents. Coordination of care for all patients, with
emphasis on obstetrics and gynecology Occasional health education
programs and community health screenings for patients, Project HOME
residents, community neighbors and staff as needed. Works
independently and collaboratively with a team What We’re Looking

among Project HOME staff. Takes a proactive stance in dealing with
the disenfranchised by addressing socio-economic needs in addition
to clinical needs. Participate in Quality and Risk Management
Committee as designated. Develop and maintain activities to achieve
Patient Centered Medical Home as required. Leadership May be asked
to develop programs to expand and support care delivery at Project
HOME Healthcare sites Education and Experience Completion of
residency in Family Medicine and Board Eligible or Board Certified
CPR certification Preferred Qualifications A minimum of 1 years'
experience providing direct patient care in a primary care setting.
Preference will be given to practitioners who have worked in an
underserved area (e.g., FQHC, free clinic) and/or who have served
patients who have significant behavioral health and social
challenges. Experience in providing obstetrical and gynecological
care. Ability to establish and strengthen relationships with the
community, medical partners, and Project HOME staff across the
organization. Excellent written and verbal communication skills.
