Overview

Working with the Medical Team, delivers medical care for specific patients with Substance Use Disorders (SUD), including formulating a diagnosis and a treatment plan and providing continued follow up patient care. Serves as subject matter expert for the facility's Medication Assisted Treatment (MAT) program.

* Provide direct primary and acute addiction care services to adult patients * Perform initial and on-going assessment of patients' medical, physical and psychosocial status * Obtain and evaluate health and medical history; perform a physical exam, conduct preventive screening procedures based on age and history * Identify medical and health risks * Complete accurate and timely documentation of observations, interventions, plan of care, referrals and follow-up * Formulate appropriate differential diagnosis based on history, physical examination and diagnostic testing * Identify needs of the individual, family or community and works in collaboration with medical and SUD team to develop individualized treatment plans. * Make appropriate referrals to other health professionals and community agencies * Reassess and modify the plan as necessary to achieve medical and health goals * Develop a patient education plan relevant to the condition and patient/ family needs * Participate in performance improvement activities as assigned * Mentors and coaches other health team members. * Orders and interprets diagnostic tests. * Writes orders for medical medications and treatments, under guidelines of licensure. * Maintains and follows patient care protocols. * Works with members of the healthcare team in educating the patients of their medical care. * Continually updates clinical knowledge and skills through consultation, formal and informal education and review of literature for the benefit of themselves as well as for the healthcare team. * All other duties as assigned.

* A current CPR/BLS card with demonstrated and cognitive skills is required. * Must possess a Master's Degree in Nursing, MD or DO. * Must have current and valid Nurse Practitioner or Advanced Practice Nurse license with at least one year experience of providing direct patient care. * Must have prescriptive authority, including DEAX * A minimum of one year experience treating patients with substance abuse disorders. * Computer Literacy * Previous experience in a correctional setting preferred.
