Short Description Provide ongoing care-coordination and case management services. Guide linkage to appropriate clinical services including medication-assisted treatment (MAT), behavioral health treatment, and social services to individuals with Substance Use Disorder. -Provide education, coordination, navigation, linkages, and support to individuals with Substance Use Disorder, mental health diagnosis, or other medical conditions. -Foster one-on-one supportive relationships built on trust and respect, and encourage engagement and retention in treatment in order for patient to reach and maintain optimal outcome -Provide continuity of care by meeting patients in various clinical and community settings including the following: home visits, emergency department visits, addiction treatment appointments, medical appointments, social service appointments, correctional visits, and therapeutic appointments. -Identify health and wellness needs, barriers and strengths including physical health, medical and addiction treatment, social support, occupational and intellectual strengths and goals, financial and housing, etc. -Perform outreach by meeting with patients in the community and act as allies and advocates for patient services -Attend Addiction Medicine clinical and programmatic conferences and meetings as required. -Engage in ongoing professional development, training and receipt of clinical supervision. -Experience Required1 year of relevant experience if candidate has Bachelor's degree in related field or 2 years of relevant experience if candidate has Associates degree or is a Certified Peer Recovery Specialist. 4 years of relevant experience, either clinical or lived, if candidate is a Certified Medical Assistant. Education RequirementsBachelors' degree in a related field, plus 1 year of relevant experience working with patients with Substance Use Disorder -OR Associates degree with 2 years of relevant experience -OR Certified Peer Recovery Specialist designation plus 2 years relevant experience -OR Certified Medical Assistant with 4 years of relevant experience -OR 4 years of relevant/lived experienceSpecial RequirementsMust be comfortable working with an underserved patient population and utilizing harm reduction models of care. -Must be comfortable working with patients in the community, attending appointments with them in a variety of seeting and ongoing advocacy work. -Excellent interpersonal and communications skills. Be willing to engage in cooperative, professional relationships with all members of the interprofessional healthcare team. -Scheduled Days / Hours: Days with some evening/weekend
