- Provide ongoing care-coordination and case management services.
Guide linkage to appropriate clinical services including
medication-assisted treatment (MAT), behavioral health treatment,
and social services to individuals with Substance Use Disorder.

- -Provide education, coordination, navigation, linkages, and
support to individuals with Substance Use Disorder -
- Foster one-on-one supportive relationships built on trust and
respect, and encourage engagement and retention in treatment in
order for patient to reach and maintain optimal outcome -
- Provide continuity of care by meeting patients in various
clinical care settings including the following: emergency
department visits, addiction treatment appointments, medical
appointments, social service appointments, and therapeutic
appointments. -
- Identify health and wellness needs, barriers and strengths
including physical health, medical and addiction treatment, social
support, occupational and intellectual strengths and goals,
financial and housing, etc. -
- Perform outreach by meeting with patients in the community and
act as allies and advocates for patient services -
- Attend Addiction Medicine clinical and programmatic conferences
and meetings as required. -
- Engage in ongoing professional development, training and
receipt of clinical supervision.Must be comfortable working with an
underserved patient population and utilizing harm reduction models
of care. - -Excellent interpersonal and communications skills. - Be
willing to engage in cooperative, professional relationships with
all members of the interprofessional healthcare team. -Valid
driver's license. - Must posess own vehicle or reliable alternate
means of transportation for use to outeach individuals
served.Experience Required1 year of relevant experience if
candidate has Bachelor's degree in related field or 2 years of
relevant experience if candidate has Associates degree or is a
Certified Peer Recovery Specialist. 4 years of relevant experience,
either clinical or lived, if candidate is a Certified Medical
Assistant. Education RequirementsBachelors' degree in a related
field, plus 1 year of relevant experience working with patients
with Substance Use Disorder - OR Associates degree with 2 years of
relevant experience OR Certified Peer Recovery Specialist
designation plus 2 years relevant experience. OR Certified Medical
Assistant with 4 years of relevant experience. Special
RequirementsScheduled Days / Hours: 8a-5p or 12 hr shifts.; License
(or Certification):CMA Preferred; License (or Certification):CPRS
