Formulates the discharge plan with patient, families and the care team based upon a needs assessment.-- Coordinates appropriate referrals to home care agencies, skilled nursing and rehabilitation centers, and community based programs. Coordinates care authorization process with insurers. Coordinates specific details of patient's hospitalization with Utilization Management department to ensure appropriate admission status.--Keywords:Discharge PlanningDischarge PlannerSocial WorkerCase ManagerExperience Required3 - 5 years health care experience preferred.Education RequirementsRequires a Master Degree in Social Work (MSW) or a licensed RN, BSN preferred License/Certification RequirementsCurrent NJ SW License, LCSW Preferred.orCurrent NJ RN License --ACM (American Case Management Association) prefferedCCMC (Commission for Case Manager Certification) preffered Special RequirementsScheduled Days / Hours: M-F, 8-4:30 pm; License (or Certification):NJ-LCSW Preferred; License (or Certification):NJ-LSW Preferred; License (or Certification):NJ-RN Preferred; Education:MSW Preferred
